License holders are encouraged to complete a course evaluation each time they complete a course through a TREC or TALCB approved education provider.  Online versions of the Commission and Board course evaluation forms can be submitted electronically on the agency websites.  PDF versions of the course evaluations should be submitted directly to your education provider.
